What Is Unilateral Hearing Loss?

Unilateral hearing loss (UHL) refers to partial or complete loss of hearing in one ear while the other ear functions normally. It can range from mild difficulty hearing soft sounds to total deafness in one ear. This condition can make it hard to locate where sounds come from, follow conversations in noisy places, or hear certain tones.

Common Causes of Hearing Loss in One Ear

There are many possible reasons why you might experience hearing loss in only one ear. Some of the most common include:

1. Earwax Blockage

A buildup of earwax (cerumen) can block the ear canal, muffling sound and creating a temporary hearing loss. Fortunately, this is one of the easiest causes to treat.

2. Ear Infections

Middle or inner ear infections can cause swelling, fluid buildup, or damage to the delicate hearing structures, leading to one-sided hearing loss.

3. Sudden Sensorineural Hearing Loss (SSHL)

SSHL is a rapid loss of hearing that occurs within hours or days, usually due to nerve damage. It’s considered a medical emergency—early treatment greatly improves the chances of recovery.

4. Acoustic Neuroma

This is a noncancerous tumor on the hearing and balance nerve. While rare, it can cause gradual hearing loss, tinnitus (ringing in the ear), and balance problems.

5. Head or Ear Injury

Trauma to the head or ear can damage the auditory system, leading to temporary or permanent hearing loss in one ear.

6. Meniere’s Disease

This inner ear disorder often causes hearing loss in one ear, dizziness, and a feeling of fullness or pressure.

7. Ototoxic Medications

Certain drugs, such as chemotherapy agents or strong antibiotics, can harm the inner ear, resulting in hearing loss.

Symptoms to Watch For

Hearing loss in one ear can show up in subtle or obvious ways. Common symptoms include:

  • Difficulty understanding speech on one side

  • Trouble locating where sounds come from

  • Feeling that one ear is “blocked” or “full”

  • Ringing or buzzing in one ear (tinnitus)

  • Dizziness or balance issues

If your hearing suddenly changes—especially within a few hours—see a doctor or audiologist immediately.

Effective Treatment Options

The right treatment depends on the underlying cause. Some common approaches include:

1. Medical Treatment

  • Earwax removal: A doctor can safely clean blocked ear canals.

  • Antibiotics or antifungal medications: Used for bacterial or fungal ear infections.

  • Steroid therapy: For sudden hearing loss, corticosteroids may reduce inflammation and improve recovery.

2. Surgery

If structural issues or growths like acoustic neuromas are present, surgical removal or correction may be necessary.

3. Hearing Aids and Assistive Devices

People with permanent unilateral hearing loss can benefit from:

  • CROS (Contralateral Routing of Signal) hearing aids

  • Bone-anchored hearing systems (BAHS)
    These devices help transmit sound from the weaker ear to the stronger one.

4. Lifestyle and Home Care

Avoid exposure to loud noises, manage stress, and maintain good ear hygiene. Regular hearing check-ups can catch potential issues early.
